Health Care Coordinator
Our Health Care Coordinator is a nurse who is available full time to assess and monitor participants throughout the day. During the participants free trial visit, our Health Care Coordinator, along with other members of our care team, assess the participant to determine what our care team needs to focus on during future stays with us.
The Health Care Coordinator is also available to give participants their medications and to monitor our participants throughout the day.
The Health Care Coordinator also serves as a family resource to help families identify other health care needs that may be needed for their loved one attending our program.
